HOW TO APPLY FOR A MEDICAL OR RELIGIOUS EXEMPTION

You may apply for a medical or religious exemption from vaccination. However, some medical and safety-sensitive positions require vaccination and are not eligible for an exemption. 

  • Team members who obtain approval for an immunization exemption will be required to wear a surgical mask and other identified infection prevention practices. Team members must be formally approved for exemption by the exemption deadline date.
  • Of note: To assess the request appropriately, team members must understand that they may be asked to submit a new application for religious/medical exemption in the future. An approved exemption does not automatically mean your exemption will be approved permanently.
  • Step 1: Access the Exemption Form:
  • Step 2: Complete the Form: The requestor will complete the form, include a written rationale for their exemption request, and submit the form to the EOH 2025-2026 Flu Campaign Smartsheet. Please note the medical request will require a practitioner to complete a section of the exemption form.
  • Step 3: Form Review by Oversight Committee: The oversight committee will review all exemption requests and make a determination. The committee considers both medical and religious exemption requests. All medical exemptions are based on CDC/ACIP contraindications and are reviewed by the committee.
  • Step 4: Notification of Decision: Team members seeking an Exemption Request shall be provided with a written response to such request. All team members granted exemptions will be expected to follow all infection control protocols listed in the confirmation email.

Please note that team members may be asked to submit a new application for religious and medical exemptions as science and regulations change. An approved exemption does not guarantee permanent exemption.
