Community Engagement Vision

To achieve health outcomes in all communities.

We recognize that every person is different, each shaped by unique life experiences, with disparate access and opportunities. This recognition enables us to better understand our team members, consumers and communities, and eliminate disparities to create meaningful change. When we engage the strengths and talents of each team member, we ensure a strong organization capable of providing outstanding healthcare to our patients and the communities we serve. 

Healthy Outcomes: Healthy outcomes means the attainment of the highest level of health for all people. This requires removing economic and social obstacles such as poverty and discrimination, and their consequences, including powerlessness and lack of access to good jobs with fair pay, quality education and housing, safe environments and health care.*

Achieving healthy outcomes requires that we:

Embed our core value of community engagement in all that we do.

Honor each person as a human being with the right to healthcare and wellness.

Build community partnerships and presence to understand and address unique needs of all communities.

Commitment to provide access to care, demonstrate compassion and promote well-being.

Upward mobility: Upward mobility is the capacity to rise to a higher social or economic position. Communities affected by social and economic inequality often don't have the resources they need to be healthy and make healthy choices.

Community engagement provides programs and services that are strategically focused on achieving health equity -- improving health and upward mobility in all communities, especially those that need it most.
